dee manufactures a variety of flatwork forms and accessories including straight, radius, and Heltzel-type flatwork forms. Our straight and radius metal flatwork forms are used in concrete flatwork placement such as sidewalks, driveways, and foundations.

Straight flatwork forms

dee's flatwork forms ensure a perfectly straight concrete placement every time.

Available in heights from 4" to 7 1/2" in 12-gauge steel, and from 4" to 24" in 10-gauge steel, dee's straight flatwork forms are offered in standard 10' lengths with either 2" or 4" bases, but can also be obtained in shorter lengths.

Radius flatwork forms

dee's radius flatwork forms ensure a perfectly true radius forming concrete placement every time.

Available in heights from 4" to 24" in high carbon spring steel, dee's radius flatwork forms are offered in standard 10-foot lengths, have 7 stake pockets, attached end angles, and lock clamp connectors included.

Heltzel-type flatwork forms

dee also manufactures a Heltzel-type form with a 2" rail for forms up to a height of 11" and a 3" rail for forms from 12" to 24" in height in standard 10-foot lengths.
